Update- My wife has been on the phone with my brother every night going over his sugar testing and taking his insulin. He is not consistently checking his blood sugar levels or taking the insulin during the day, but she reminds him each evening and he tests his blood sugar and takes his overnight insulin when she reminds him. I think this is about as good as it will get with him.
